Background Check Privacy Notice

Community Intervention Healthcare Services (CIHS) in compliance with State requires criminal background checks prior to the final employment of any personnel. These results are handled in a manner that protects the applicant’s privacy. These obligations are pursuant to the Privacy Act of 1974, Title 5 United States Code Section 552a, and Title 28, Federal Regulations (CFR), Section 50.12.

Your fingerprints will be used to check the criminal history records of the FBI. Any person seeking employment with CIHS is required to make a full disclosure of any prior criminal record, misdemeanor or felony. Any applicant who knowingly falsifies information regarding a prior conviction of a misdemeanor or felony shall forfeit his/her office (Background check Policy).

Applicants will be granted time to provide CIHS with documentation regarding the final outcome of any and all charges that appear on their background check results. Staff members will receive 30 days to provide the proper documentation.

The procedure to request a change, correction or update of an FBI criminal history record should be done through the FBI and those instructions are established in 28 CFR 16.34.

CIHS will only use the background check results for the requested purpose and cannot disseminate the record outside the receiving department, related agency, or other authorized entity.
